I have my local probe that will not return to healthy state and already tried both updating the latest PRTG and the registry hack for NoMacScan with still no luck.

Can you let me know what is next other than a complete reinstall?

We need some more information (log files) from your PRTG to give you a precise answer. Please open a support ticket so we will continue troubleshooting there. Please refer to this KB article in the ticket.
