How to set limits with list of values?

Hi! I have snmp sensor which return one of the following values: 1, 2, 3, 4. How to set up sensor limits, to make warning if value is "1" and "3", make alert if value is "4", and make 'ok' if value is "2"? For each of this values there is a "lookup", so 1 - unknown, 2 - batteryNorma, 3 - batteryLow, 4 - batteryInFaultCondition
